Membership Form

Membership dues are $30.00 per year, July 1 - June 30, payable by check made out to NCBA.
A late fee of $5.00 will be added after August 1.
If you join AFTER September 30, you may still attend the annual convention. However, a $5.00 shipping and handling fee (in addition to the $5.00 late fee) will be assessed if you wish to receive the convention brochure; and your convention class choices will be assigned after all other class requests are filled.
Only members of North Carolina Basketmakers' Association are eligible to register for and attend OTHER NCBA-sponsored functions, such as Fall Seminars.

Please print out this page and print or type in the requested information. Be sure to include your membership number if you are a current member and note if this is a new address . Mail, along with your check for $30.00 ($35.00 after August 1, $40.00 after September 30). No pro-rated dues. Send to:
